Heartland Bank Quarterly Survey Sweepstakes
Official Rules
NO PURCHASE NECESSARY. PROMOTION BEGINS on the 1st day of each quarter and ends at 11:59PM E.T.Time on the last day of each quarter (the “Promotion Period”).
1. HOW TO ENTER. You will automatically be entered into the sweepstakes when you respond to the Personalized URL Onboarding survey available via your personalized letter from Heartland Bank. For survey entry, limit one entry per month. Alternatively, you may also enter by sending a plain piece of 3" x 5" paper hand-printed with your complete name, address, ZIP code, birth date, and daytime and evening telephone numbers to: Heartland Bank, Marketing Department, 430 North Hamilton Road, Whitehall, OH 43213. No mechanically reproduced entries permitted. Sponsor is not responsible for incomplete, lost, late, damaged, illegible, defaced, mutilated, incomplete, or postage-due mail/entries or scanner/computer failures, errors or malfunctions of any kind whether human or technical in nature. All entries become the property of Heartland Bank and will not be returned. Sponsor reserves the right to terminate the sweepstakes at its discretion following the close of any Promotion Period.
2. SELECTION OF WINNERS. One winner will be determined from all eligible entries received each month in a random drawing to be conducted within 7 days of the end of the month. Winner will be notified by mail and/or telephone.
3. PRIZE. One (1) Quarterly Prize: One winner each quarter will win a prize worth approximately $100. Prize is nontransferable and cannot be redeemed for cash.
4. ODDS OF WINNING. Odds of winning are determined by the number of qualified entries received each month.
5. ELIGIBILITY. The sweepstakes is open to legal residents of the 50 United States and the District of Columbia, age 18 or older, except employees (and their immediate families & household members) of Sponsor, their respective affiliates, agents, judges, and advertising and promotion agencies.
6. WINNER VERIFICATION & PRIZE DELIVERY. Prize may be awarded to alternate winner if prize notification or prize is returned as undeliverable. Winners agree to use of name, address, likeness, and/or prize information for promotional purposes in any medium without additional compensation to the extent permitted by law, any such use with respect to minors to be subject to prior approval of parent or legal guardian.
7. GENERAL.This offer is void where prohibited or restricted by law and subject to applicable federal, state and local laws and regulations. Sponsor and judges not responsible for late, lost, damaged, postage due, incorrect or misdirected entries or mail. By participating in this Sweepstakes, each entrant accepts the conditions stated in these Official Rules, agrees to be bound by judges’ decisions and warrants that s/he is eligible to participate. By accepting a prize, each winner agrees to release Sponsor, their respective directors, employees, officers, and agents, including without limitation their advertising and promotion agencies, from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of prize or participation in any prize-related activities. If for any reason, the sweepstakes is not capable of running as planned, including without limitation, any suspected evidence of tampering or technological corruption or if any portion of the sweepstakes is compromised by virus, bugs, worms or unauthorized human intervention, an insufficient number of qualified sweepstakes entries, or any other causes beyond the Sponsor’s control which, in their sole opinion, corrupts, threatens or impairs the administration, security, fairness, integrity, or proper conduct of the sweepstakes, the Sponsor reserves the right to cancel, terminate, modify or suspend the sweepstakes at itssole discretion.
8. WINNERS LIST. To obtain the list of prize winners, send a self-addressed, stamped envelope by the last day of the following Promotion Period to: Heartland Bank, Marketing Department, 4430 North Hamilton Road, Whitehall, OH 43213.
